Carer Assessments

If you are a Carer providing subtantive care to a friend or relative then you are entitled to ask for a Carer's Assessment .

What is a Carer's Assessment?

A Carer's Assessment is when the Carer is able to talk about the care they provide and how it is affecting them. This can enable some things to be identified that may be able to be put in place to help the Carer.

Who carries out the Carer's Assessment?

Often it is the Social Work department who carry out the assessment, although it can be someone in the NHS, an Occupational Therapist, or Voluntary Agency such as a Carer Centre. The first part of the form could also be done as a self-assessment, but generally we would recommend that someone else talks through the questions with the carer as it helps to identify areas where help may be provided.

How do I get A Carer's Assessment?

No matter who completes Section 1 of the form, in order to be assessed for any knid of assistance, the form must be returned to your local Social Work Office, who will then contact you to arrange an appointment.
